Your First Visit
Welcome!
Prior to your initial consultation, please download and fill out a New Patient Intake form. If you are bringing your child to see Dr. Cimperman, please download and fill out a Pediatric Intake form.
If possible, email the completed form to the doctor prior to your appointment (at least 24 hours in advance is appreciated). In the subject line, write “New Patient Intake” and your name.
Bring the following items with you to your appointment:
- A paper copy of the Intake Form (even if you emailed it to the doctor)
- Copies of any lab tests that are recent (within the past year) or relevant (diagnostic of conditions you wish to discuss with the doctor)
- Medications, vitamins and supplements you are currently taking
Come to 19 West 34th Street, between Fifth and Sixth Avenues (across from the Empire State Building). The closest subway stop is "34th Street" on the B, D, F, V, N, Q, R or W trains. Take the elevator to the Penthouse (PH) level and check in with the receptionist when you arrive.
Dr. Cimperman only sees one patient at a time and her appointments start on time in 99 percent of cases. Please arrive on time and expect to have her full attention during your appointment.
Because Dr. Cimperman sees only one patient at a time and spends a significant amount of time with each patient, late arrivals, last-minute cancellations and missed appointments affect her significantly. If you need to reschedule or cancel your appointment, please call 646-234-2918 as soon as possible. Note that there is a $50 cancellation fee for missed appointments and those canceled or rescheduled with less than 24 hours notice (for Monday appointments, please call by 5:00 p.m. on Saturday).
Supplements
Dr. Cimperman does not sell supplements but established patients can order products she recommends from Emerson Ecologics.
To order from Emerson, follow the Virtual Dispensary link below and enter the access code “34w19s”. Then you will be prompted to create an account or log in to an existing account. Once you log in, you will be directed to a page that contains an A-to-Z list of products hand-picked by Dr. Cimperman. You will also have the option of using the search box at the top of the page to look for other products. To minimize the number of products listed and make the form easier to use, the doctor has added only one size of each product to the list. Note that other sizes are often available and can be found by searching for the product by name. If you are having difficulty finding products by name, you can also search alphabetically under each manufacturer.

Alternatively, you can call the customer service department at 1-800-654-4432 to set up your account and place orders.
Orders received by 6:30 p.m. EST are shipped directly to you on the same day for delivery in 1 to 3 business days anywhere in the United States. Shipping costs $6.50 for orders below $40.00 and $9.00 for orders totaling $40.00 or more.
Like pharmaceutical medicines, natural medicines can have adverse effects when taken incorrectly. For optimal results, follow the doctor’s recommendations and choose supplements from reputable manufacturers.
Seek products that list the following information on the label:
- The name and address of the manufacturer
- A lot number or batch number
- Expiration date
- The scientific name, quantity and part of any plant ingredient
Also, to ensure that products have been tested by an independent lab, look for seals from independent organizations like
- The United States Pharmacopeia
- The National Nutritional Foods Association
- Consumer Lab
- National Sanitation Foundation International
Certification guarantees that products contain what they are labeled to contain. However, it doesn’t ensure that manufacturers started with the highest quality raw ingredients or tested them in clinical trials.
